Prescription Refills
For a quick turnaround on refill requests, please bring your medication list with you to each appointment so that it can be handled by our provider and staff at the same time as your visit!
Forms
Please allow our office staff a minimum of 48 hours for completion of forms. We will notify you as soon as the form has been completed.
Release of Medical Records
It is our goal to provide you with a copy of your records at the time of your request, but please understand that it could take up to 24 hours to complete your request after a release form is signed.
